Is it true, without exception, that if P, Q, R are points contained by plane X and also by plane Y, then X is the same plane as Y?

False.  Whenever two planes cross the intersection makes a line that is common to the two planes.  Those three points, P, Q and R, could all be on that line common to the two different planes X and Y.
