Lysosomes are formed by the Golgi Apparatus.  Proteins made by ribosomes on the Rough Endoplasmic Reticulum are packaged in a membrane sack (vesicle) and sent to the Golgi.  There, various modifications are made, and then the digestive protein enzymes of the lysosome are packaged in the appropriate membrane and the lysosome is pinched off of the Golgi
